මෙය කොවිඩ් – 19 ආසාදනය වී තමන්ගේ නිවෙස්වල ම ප්‍රතිකාර ලබන රෝගීන්ට (නිවාසවල සිට නිරෝධායනය වන පළමු පෙළ ආශ්‍රිතයන් නොවේ) ලැබෙන සේවාවන් පිළිබඳ ව කරනු ලබන ස්වාධීන සමීක්ෂණයකි.
ඔබ කොවිඩ් – 19 ආසාදනය වීම නිසා නිවසේ සිට ප්‍රතිකාර ලබන කෙනෙක් නම් / ඔබේ පවුලේ කොවිඩ් – 19 ආසාදනය වීම නිසා රෝගී වූ කෙනෙක් සිටී නම් / ඔබ දන්නා හිතවතෙක්ට කොවිඩ් – 19 ආසාදනය වී ඇත්නම් මෙම සමීක්ෂණයට සහභාගි වන්න.

Assessing information on COVID-19 infected individuals undergoing home-based management
This is an independent survey aimed at exploring the services provided for home-based treatment and management of individuals diagnosed with COVID-19. (This survey does not apply to individuals who are in quarantine due to being identified as a first line of contact with a COVID-19 patient).
You can participate in this survey if you are an individual who has been diagnosed with COVID-19, but is being treated at home / if a family member has been diagnosed with COVID-19 / if a friend or relative has been diagnosed with COVID-19.
We kindly request your maximum support in this regard.
